What you'll need
- The name of the medicine, vaccine or product, and the batch number if you have it
- What happened, and when it started
- The age and sex of the person affected
- Your phone number, so ZFDA can contact you if needed

What happens next
- You send your report You get a reference number. Keep it to track your report.
- ZFDA reviews it Pharmacovigilance officers assess the report and may contact you for more details.
- You get feedback Use your reference number on this site to see the feedback on your report.
